About the Test
Test Availability
Boys Town National Hospital allows a limited number of individuals to take the EIPA Performance Test each month. This ensures they will be able to evaluate candidates and send reports within a reasonable time frame. The Center for Outreach Services at OSD is allotted a certain number of testing slots within a given month which determines how many tests we can proctor.
Application for Testing
A
completed application must be received
to reserve a testing slot, schedule a testing date or be placed on our waiting list. Applications must be postmarked by the first of the month one month ahead of the month a candidate would like to test (e.g. by June 1
st to test in July). Applicants will be contacted to set a testing date and time. Download an
EIPA Performance Test Application form to view with
Adobe Reader and print, complete and send.
Individuals who live in Ohio or work in Ohio public schools have priority for EIPA testing slots at OSD. Out-of-state individuals can send in an application to be placed on a waiting list. If there are slots remaining for a given month, individuals on the waiting list will be contacted a month prior to see if they would like to schedule.

Payment
All candidates must pay $250 on the day of testing, payable to Boys Town Research Hospital, using check, money order, purchase order, or credit card (Visa, MasterCard, Discover, or American Express).
Individuals from out-of-state (who do not live in Ohio or work in an Ohio public school) must pay an additional $75 proctor fee. Payment can be made using check, money order or purchase order payable to the Ohio School for the Deaf on the day of testing. The proctor fee is currently waived for individuals who live in Ohio or work in Ohio public schools.
